This picture shows the Gardiner Eastbound off-ramp serving Bay, York and Harbour Streets.

If you look closely you can see the overhead gantry on the ramp which splits the exit: the loop around to York and Harbour Streets is on the right, traffic to the ramps' left proceeds straight along down to Bay St.

In addition, if you study the main Eastbound carraigeway to the bottom right you can see one of the numerous on-ramps joining the freeway from Lake Shore Blvd. It is sandwiched in between the carraigeway and the exit referred to above.
