Very nice example of a Highway trailblazer - which to be specific is the lower shield directing traffic towards Toronto. This blue format is of course unique to the QEW, trailblazers of all other Kings Highways are green. Another interesting point of note is the uni-lingual "TO" as opposed to "TO / VERS" seen on (presumably) newer trailblazers.
Above the trailblazer pointing us towards Niagara is what James Lin on his website Highway Route Markers calls a "Trailblazer Junction Sign" - also decked out in QEW uniform. The difference between the two seems to be a somewhat tenuous one in this particular case. Turning right brings you on to a street which has direct access to a QEW on-ramp. But the street we are currently on also has direct access - to a Niagara bound ramp. Shouldn't the arrangement should be reversed with the highway trailblazer on top and the Junction trailblazer below ?
